Dear SPA 8 Community Stakeholders,
The Los Angeles County Department of Public Health would like to invite you to the Service Planning Area 8 Regional Network Meeting which will be held Thursday, January 25th 2018. The Regional Network Meetings are convened to encourage networking, share resources, examine best practices, and improve coordination amongst key stakeholders.
The upcoming meeting will focus on the Whole Person Care program. Whole Person Care is a five-year initiative that builds upon existing infrastructure to create a more integrated delivery system and provides comprehensive and coordinated services to the most vulnerable residents in in LA County. The program helps to coordinate physical health, behavioral health and social services across the County to deliver seamless, coordinated services to individuals who are homeless, justice-involved, have serious mental illness, or severe and/or persistent substance use disorder or medical issues.
This meeting is a great opportunity to learn more about this program and engage in a discussion on how to get involved to help create more equitable communities in SPA 8 through Whole Person Care. There will also be opportunities for resource sharing and networking with other SPA 8 stakeholders. Please be prepared to share any resources with fellow SPA 8 network members and engage in networking.
